Human Chorionic Gonadotropin
 
Studies have disproven the notion that hCG, administered orally or via injection, has any effect on appetite, fat mobilization, or satiety. In addition, it has been associated with serious adverse effects, including ovarian hyperstimulation, hypercoagulability, and multiple pregnancies. However, hCG continues to be promoted on a variety of Websites as a potential solution to obesity. The US Food and Drug Administration has issued warnings to manufacturers and consumers noting that these products are unapproved for use. There is no human evidence supporting the use of hCG for weight management.
 
Green Coffee Beans
 
Green (unroasted) coffee beans have likewise received plenty of attention and are sold as extracts and capsules on multiple Websites. Green coffee beans are reported to provide lower levels of caffeine and higher amounts of chlorogenic acid than do roasted coffee beans. Chlorogenic acid is believed to affect glucose metabolism and to inhibit fat accumulation by altering adipokine levels.
 
Published human studies on green coffee beans are of short duration and poor quality, reporting weight loss of around 1.5-2.5 kg over 1-2 months using extracts at doses ranging from 80-1000 mg of extract daily.[10] Orally, green coffee bean is well tolerated, and adverse effects are those caused by excess caffeine intake, including headache, diuresis, gastric distress, vomiting, insomnia, anxiety, and arrhythmias. Use of green coffee beans concomitantly with other caffeine-containing products, such as cocoa or tea, may increase the risk for caffeine-related adverse effects. High caffeine intake in postmenopausal women has been linked to osteoporosis. There is currently insufficient evidence supporting the use of green coffee beans as a weight-loss product.
 
Green Tea
 
Compared with green coffee beans, more robust evidence, including clinical trials of at least 12 weeks' duration, is available on the effects of green tea for weight loss. A recent Cochrane systematic review examined 8 studies conducted in Japan (1030 patients) and 6 studies conducted in other countries (532 patients). Green tea was provided either as a beverage or as catechins (purified extract from green tea) and compared with placebo. The studies were considered to be of moderate quality.
 
In aggregate, weight loss in obese patients over an average of 12 weeks of daily ingestion was minimal and did not achieve statistical significance. The authors concluded that it was not likely to be clinically important. Adverse effects were mild. However, green tea has been associated with other beneficial, mainly cardiovascular, outcomes because of its antioxidant effects.
 
Açaí Berry
 
Açaí is a palm tree growing in the north of South America; its berry is consumed raw or as a juice. Its antioxidant content is believed to be higher than that of other berries, such as strawberries, blackberries, and blueberries. Because of this, it has been advocated for use in inflammatory conditions and prevention of cardiovascular disease. In food manufacturing, the açaí berry is used as a colorant.

Treatment significantly reduced the severity of pruritus (itching) in adult patients with eczema.
 
Açaí berry has been marketed as a weight-loss product in the form of pills, capsules, and juice. A small recent pilot study suggests a benefit for metabolic syndrome. However, there is no clinical evidence supporting its efficacy in weight loss.
 
Raspberry Ketone
 
Raspberry ketone is one of the most expensive flavoring agents in the food industry and is found in low concentration in red raspberry fruits. It is extracted from such sources as peaches, grapes, apples, and rhubarb, which have higher concentrations. There is no dose standardization for raspberry ketone. It has been promoted widely as a weight-loss agent owing to celebrity endorsement.
 
Raspberry ketone has been reported to have antioxidant effects in rats, as well as to promote lipolysis in vivo. It was reported to reduce low-density lipoprotein cholesterol and increase high-density lipoprotein cholesterol in an animal model.  However, antilipemic and antiobesity effects on humans have not been demonstrated in clinical trials.

What are the conservative treatment options?
Core conservative treatments for all patients include education and exercise, with weight loss for those who are overweight or obese.
Exercise is essential for all patients, irrespective of disease severity, age, comorbidity, pain severity, or disability. Local muscle strengthening and general aerobic exercise are recommended, whereas aquatic exercise may be better for people with serious mobility and functional limitations or those who cannot exercise on land. Although exercise is beneficial, sports that involve twisting and high impact activities such as running should be avoided. Instead, advise younger active people who develop osteoarthritis after a knee injury to modify their activity and participate in low impact sports such as cycling, rowing, swimming, and deep water running.


What is the role of surgery?
The American Academy of Orthopedic Surgeons recommends that arthroscopic lavage or debridement (or both) and meniscal resection be performed only in patients with mechanical symptoms, such as sudden onset of inability to fully extend the knee or repeated disabling catching or locking. This operation has no demonstrable effect on pain in osteoarthritis of the knee, with the latest studies showing similar benefits to optimized physical therapy and medical treatment.

Weight loss statements from the New England Journal of Medicine (and Kaizen's 2 cents!)

1. Moderate lifestyle changes can be as effective as weight loss medications. (Easier said than done!)
2. Diets do help people lose weight, but attempting it and doing it are two different things. (Absolutely true)
3. Physical activity can have health benefits even in the absence of weight loss. (I have written about this many times in the past)
4. Providing actual meals or using meal replacements works better than general advice. (This is why we use a registered dietician)
5. Weight loss prescriptions and bariatric surgery can help with long term weight loss. (Qysmia has FDA approval for long term use, but only 305 of insurance companies will pay for it)
6. Small changes in diet/exercise will  not produce long term weight loss. (We don’t know why, but over 85% of people who lose weight will gain it all back no matter what kind of program they are on)
7. Realistic weight loss goals WILL NOT keep people motivated. (I don’t believe this statement-accountability will help keep people motivated. That’s why The Biggest Loser is so effective, until the people no longer have access to that kind of support)
8. Slow weight loss is not as good as rapid weight loss using very low calorie diets. (I don’t know where they got this from, but it is absolutely flawed. We know that fad diets do not work long term unless you plan to live on 500 calories the rest of your life. All rapid weight loss does is give you a quick fix, which is not only not sustainable, but also leads to decrease muscle mass and preservation of fat----“normal weight obesity”)
9. Sexual activity does not burn enough calories to lose weight. (Agreed)
10. Skipping breaking has no bearing on weight loss. (I have to disagree. Eating a nutritious breakfast after not eating for 8-12 hours is very beneficial and sets the tone for the rest of the day)
11. Adding fruits and vegetables do not help weight loss. (Yes a calorie is a calorie but when we recommend adding fruits and veggies, we are assuming that you are not eating chips, candy and soda!)

Several months ago I wrote about the ineffectiveness of SENSA. It consists of multiple ingredients that are formed into sprinkles that are called “tastants” that you sprinkle on top of your food which would then allow you to lose weight without “dieting, counting calories or sacrifice”.
The company claimed they had clinical studies that proved most people lost 30 pounds while taking it. The only problem is that the studies did not meet FTC guidelines.
To make a long story short, they were fined almost $1,000,000 and told to set up a settlement fund of $9,000,000 after losing a class action suit.

Weight loss: pounds vs. inches

Many of our patients get discouraged when the scale does not show a lot of rapid weight loss. Our recommendations suggest losing weight slowly over long periods of time in order to develop healthy lifestyle changes.
Most fad or gimmick diets that lead to rapid weight loss basically are achieved by water loss and muscle loss (not to mention up to 97% failure rate in keeping the weight off!). Our goal, at KAIZEN TOTAL WELLNESS is to teach you how to make muscle and burn fat. This does not occur rapidly. We also encourage our patients to drink plenty of fluids (there goes the diuretic effect!)
 
In order to achieve both weight loss and a flatter belly, one has to include strength training and make sure enough protein (and fat) is included in the diet. Building muscle will not only burn fat but burn more calories.
Since muscle weighs more than fat, some people will NOT see changes on the bathroom scale even though they continue to lose inches.

Nutritionist's view of weight loss: A visit down memory lane!

Let's take a quick "stroll" down memory lane of all the 'newest weight loss miracle' we've seen over the recent years.....


Does anyone remember Metabolife? It worked pretty good and was over the counter. The only problem is that it was not safe and the FDA finally took it off the market when patients started hemorrhaging from the brain.
How about some “prescriptions” that were originally blessed by the FDA? Phen-Fen was and still is the BEST product I have ever seen for weight loss. It worked on everyone—and not just 5-10 pounds. How about an average of 50 pounds lost! Oh, did I mention the heart and lungs complications which took it off the market? Meridia was expensive and was taken off the market in 2010 due to heart disease. And it didn’t even work! Xenical (Alli) is another loser. The average weight loss with this was 5 pounds over 12 months and the side effects were awful (unless you are a fan of uncontrollable diarrhea!) Of course we do have some effective drugs for weight loss which include phentermine, Topamax, and naltrexone. Some medications, like the diabetes pill metformin have a nice side effect of mild weight loss and is very safe. Some people think it can also prevent cancer, but I would not recommend using this drug off label at this time.
 
Remember the Atkins Diet and all its copycats?  It will work but it is the RARE person who can maintain a diet like this for more than a few months. If you don’t get bored and sick of all the high fat food, then the constipation will get to you.
Grapefruit diet? Anyone can lose if that’s all they eat! Of course, if you also take Simvastatin or Lipitor, you could also lose your life.
We don’t have to discuss HOODIA since it has already been pulled off the shelves by the FDA.
Recently we discussed raspberry ketones. Sorry, it doesn’t work (read my recent blog about this scam) Fat busters like L-Carnitine. Sure it will work, but only if you have an L-Carnitine deficiency. Since this affects roughly 300 people in the USA, I doubt the odds are in your favor of losing weight. Hydroxycut? FDA removed it for liver toxicity only to be repackaged and placed back in the stores. DHEA? Sorry, no proof and safety is not established in humans. Bitter Orange? Hypertension, heart disease and glaucoma outweigh any potential benefits. (No pun intended) Caffeine? This only seems to work when used with ephedra. Unfortunately this can be addicting and cause heart disease and insomnia. Calcium? Not only is this false, but we now believe that taking more then 500 mg/day may increase your risks of heart disease and kidney stones. Oh, by the way higher doses do not seem to prevent osteoporosis! Chitosan? Worthless. But it does appear to be safe Chromium? Sorry, it does not work. And no, it does not help diabetics either!
Diuretics? Sure loss of water will cause weight loss. Unfortunately it can also lead to dehydration and metabolic imbalances that can be fatal unless supervised by a physician. The weight loss is ALWAYS TEMPORARY as the only way to lose weight permanently is to make muscle and burn fat. Ephedra? See bitter orange and Metabolife
Laxatives? Temporary weight loss and unsafe, not to mention addicting
HcG? This works but is so dangerous, the FDA is about to take it off the market. Please read prior blogs for more information.
 
Now that we've taken a good old stroll down the memory lane of weight loss "miracles," I'm going to share the REAL secret to weight loss and maintaining a healthy weight!
There is no secret.
The ONLY way to lose and keep off weight is to change your lifestyle. Learn how to eat healthy and NOT DIET. Become addicted to physical activity. Exercise can be boring, so pick several actives you like and stick with it.

Rasberry Ketone

 

 

It is being promoted to cause weight loss in people by promoting fat breakdown.
 
It is found in very small amounts in the fruit of raspberries (provides the aroma).The ingredient in supplements may, however, be synthetic, due to lower costs.

It has been found to be safe as a food additive, but no safety studies at amounts in supplements --100 mg or more per day. (In comparison, 1 pound of raspberries provides about 1 mg of ketone.) Due to lack of information about possible metabolic and other effects, people with pre-existing health conditions (diabetes, high cholesterol, high blood pressure, etc.) should be monitored by a health care professional when using raspberry ketone. Be aware that raspberry ketone is different from the supplement raspberry leaf.

 If  you use raspberry ketone, use it with some caution --and be aware that the quality of supplements can suffer when companies rush to meet a new demand.

Please be aware that there is no such thing as a supplement to break down fat. In order to burn fat, one must eat smart and exercise to MAKE muscle. Remember that if stuff like this really worked, there would be no fat people!

Best Diet? The One You'll Stick To

 

A study in The Journal of the American Medical Association suggested that it’s whether you stick with whatever diet you choose that makes the difference. In this study, overweight and obese adults were assigned to follow the Atkins diet, the Ornish diet, Weight Watchers, or the “Zone” diet. After one year, nearly half of the participants had dropped out of the study. But those who didn’t lost similar amounts of weight (about 5 to 7 pounds each, on average). People assigned to the Atkins and Ornish diets were more likely to drop out of the study, suggesting that many people found these plans too extreme. The Kaizen Total Wellness weight-loss program is a sensible eating approach, not too restrictive, and focuses on eating the right balance of protein, carbohydrates and fats in right-sized portions 5-6 times a day.

The rapper, who says he once tipped the scales at 460 pounds, told CNN that he's lost 100 pounds and counting.
"I think I weighed about 450, 460 at my heaviest," he said, explaining that his hip-hop success encouraged him to over-indulge. "[I thought,] I'm rich now, I can go to Mr. Chow's and eat me all the lobster and steak I want… And I always took pride in being fat, that's why my name was Fat Joe."
The impetus for the lifestyle change first came in 2000, when fellow MC Big Pun died from a heart attack. In the last year, he told CNN, six of his friends -- all around his weight -- died of heart attacks.
"I realized at a certain point, all my big people were dying," he said. "I couldn't see a clearer picture, what's the difference between me and him, of me being in a casket?"
He's since hit the gym and changed his eating habits, eating healthier foods and smaller portions more frequently. The weight loss has led to other health benefits, as well.
"I was diabetic for 16 years," he revealed. "Being that I lost weight, no more diabetes."
But don't expect a name change: at any size, the "Lean Back" rapper says he's still Fat Joe.”

Hoodia a Miracle Weight Loss Drug: Another Fairytale!

 

 

Many people do not remember the HOODIA craze from a few years ago. But back then people swore by it. For those you you looking for an easy way to lose weight, it simply does not exist. To this day, the only way to lose and keep weight off is by learning how to eat SMART and HEALTHY and to EXERCISE DAILY. Maybe in a couple of years, the title of this blog will be THE COOKIE DIET or HCG!
 
Hoodia (Hoodia gordonii), is a rare cactus-like succulent plant that grows in the high deserts of the Kalahari on the border of South Africa and Namibia.
 
The drug company Pfizer began developing P57 as a prescription weight-control agent. However, in 2003 Pfizer discontinued clinical development of P57. A specific reason for this decision was not disclosed. Subsequently, the consumer products company, Unilever (which makes Slim-Fast) announced that it had acquired the rights to develop and market hoodia as an ingredient in its weight-loss products.  However, Unilever abandoned these plans in 2008 following a clinical trial of drink-based hoodia extract. In that clinical trial (published in 2011) healthy, overweight women were given, for 15 days, two servings per day of a yogurt drink that contained either 1,110 mg of hoodia extract or placebo (Blom, Am J Clin Nutr 2011). Although women in both groups lost some weight (about 2.5 lbs) and reduced their calorie intakes, those given hoodia did not eat less or lose more weight than those given placebo. Those taking hoodia experienced significantly more side effects (nausea, vomiting,and skin sensations) than those given placebo and also experienced significant increases in blood pressure and heart rate.
 
Hoodia, like other botanicals, may also be contaminated with pesticides if not grown organically.
Safety studies have not been published with hoodia or P57. Women given 1,110 mg of hoodia extract twice daily in a yogurt drink for 15 days experienced significantly more side effects (nausea, vomiting,and skin sensations) than those given placebo and also experienced significant increases in blood pressure and heart rate. (Blom, Am J Clin Nutr 2011).
 
Be aware that many supplements with hoodia contain other weight loss ingredients with potential side effects. Examples are bitter orange and caffeine, which can elevate heart rate and blood pressure, and cause nervousness and sleeplessness.

Is there really going to be a FAT TAX?

Well, it looks like Denmark is going to tax foods high in saturated fats. What’s next....a sugar tax? Maybe we can tax oxygen!

As much as we at Kaizen Total Wellness believe in eating healthy, we do not believe taxing or forcing people to change will really work. People have to want to change their lifestyles or they will never be successful.
Sure we tax cigarettes, but I’ve never seen a patient quit who still loved to smoke-you have to HATE smoking and truly want to quit. But an excise tax on food?

“One of this country's leading weight loss experts has commended Denmark's decision to become the first country in the world to impose a so-called ‘fat tax' on certain foods.

According to Prof Donal O'Shea, the obesity epidemic is ruining so many lives, 'it has to be attacked all-out'. And he predicted that a 'fat tax' was something that would eventually be considered in Ireland.
In an effort to reduce the unhealthy eating habits of its citizens, the Danish government has introduced a tax of around €2.15 per kilogram of saturated fat in a product. This means the cost of foods high in these fats, such as butter, oil and certain processed foods, will rise. For example, a small pack of butter will increase in price by about 30c. The tax was approved back in March and came into effect at the weekend.
Prof O'Shea, a consultant endocrinologist at St Vincent's Hospital and St Columcille's Hospital in Loughlinstown, Dublin, described the tax as a "great idea" and one which he would favour.
"Anything that discourages unhealthy eating is good. In general, encouraging healthy eating is the preferred option rather than discouraging unhealthy eating.
"However, the obesity epidemic is killing or ruining so many lives that it just has to be attacked all out and with the same ferocity that the likes of Burger King peddle the high fat, high salt options to young people," he told Irishhealth.com.
Last week, responding to a Dáil question, Health Minister, Dr James Reilly, confirmed that the Special Action Group on Obesity, which Prof O'Shea is a member of, "is not considering the introduction of tax on high fat, salt and sugar foods at this juncture".
However, Prof O'Shea explained that this is an issue the group will come back to. "I think it's a step by step approach. There are other issues first like the tax on sugary drinks," he said. He noted that shockingly, teenagers in the US today drink more calories than they eat.
"If we can impact on that as a first line and then follow up with the fat tax, that would begin to address the issue. You really do have to attack the problem at multiple levels on the calorie intake and physical activity front," he pointed out.
Meanwhile, Prof O'Shea said that he was delighted with Minister Reilly's confirmation last week that he intends to write to all fast food operators in Ireland to ask them to include calorie details on their menus.
"Calorie posting will probably have to be legislated for, but a voluntary scheme to start with would be great and the public sector must get involved early," he added.”

Top 10 Tips for Preventing Weight Gain

 

This article was recently seen on the internet and I thought it was appropriate since it is basically our philosophy: Some tips are obvious, but others like #9,8,7,5 and 1 are also very important.

Shedding some weight can be hard. For many men, keeping it off is even harder. Diets become stale and depriving. Training becomes overly laborious and boring. Follow these 10 tips to keep off the weight that you've worked so hard to lose.

No. 10 - Throw out junk food
No matter how disciplined you are, if junk food is there, you'll eventually eat it. All it takes is a long day at work or a rushed meal to push you toward grabbing the quick fix. Do a quick search through your house (cabinets, refrigerator, under your pillow, and so on) and get rid of unhealthy snack food. If your spouse, significant other or kids have junk in the house, throw their stuff out, too. They'll thank you later.


No. 9 - Stay hydrated
Consuming a couple cups of water before you eat will fill up your stomach and cause you to eat less. Research has shown that people using this strategy lose more weight than people who don't. If you don't like water, try adding a lemon or lime to it or try drinking a zero-calorie beverage with a little more flavor like green tea or POWERADE ZERO. The important thing is that you're staying hydrated and filling up your stomach with liquid while not taking in additional calories.


No. 8 - Get a dog

A big part of keeping weight off is maintaining an active lifestyle. It sounds ridiculous, but research has shown that people tend to become more active when they have a pet. Letting your dog take you for a walk at night will get you off the couch and keep your hand out of the junk food that you should have thrown out already.


No. 7 - Eat breakfast
Your metabolism plummets while you sleep; you go into a state of catabolism (read: breakdown). The best way to jump-start your metabolism and give your body the fuel it needs for the day to come is to eat a quality breakfast. At a bare minimum, your breakfast should have at least one serving of fruits and vegetables, a lean protein source and a glass of water. You can also add a quality whole-grain carbohydrate source, such as steel cut oats, to your breakfast.


No. 6 - Avoid sugars, pasta and white bread
When you take in a whole-grain carbohydrate source, it takes time for your body to break down the food. As a result, you receive a slow, consistent supply of food, similar to a time-release drug. On the other hand, foods like pasta, white bread or anything high in sugar are rapidly digested and give your body a quick supply of a ton of energy. Since your body doesn't need all of this energy at once, it defaults to storing it as fat. Overall, these foods have little nutritional value and generally lead to metabolic damage and fat storage.


No. 5 - Learn how to cook
Short-term diet interventions never work long-term. In order to keep weight off, you need to change your lifestyle to make it conducive to keeping weight off. This sounds simple, but for some men it can be a rough transition. If you associate healthy eating with bland foods, you probably won't continue to eat healthily for too long. Learning how to use spices and prepare foods in different ways that you enjoy will help ensure that you're eating the right foods for the rest of your life. You can start by perusing the grocery store for prepackaged spices like Cajun, Italian seasoning, Montreal steak and/or chicken, and seasoned salt. Eating healthily does not need to be boring.


No. 4 - Lift weights
Muscle has a significantly higher metabolic rate than bone and fat. Simply put, this means that it takes more energy (read: calories) to fuel muscle than it does other tissues within the body. By lifting weights and adding some muscle mass, you keep your metabolism and the number of calories you burn at rest high. This is one of the most overlooked aspects of a quality fat-loss program and is essential as part of a weight-loss maintenance program.


No. 3 - Eat every 2 to 3 hours
One of the best things you can do to keep weight off is to set up an eating schedule and stick to it. Eating regularly tells your body that you are going to supply it with a consistent supply of quality nutrients and that it doesn't need to hoard anything. It will also ensure that you don't binge eat because you're "starving." Figure out when you usually wake up in the morning and when you go to bed at night, then plan meals every two to three hours in between. Remember that keeping weight off is about keeping your metabolism high. Skipping meals lowers your metabolism, too, so stick to your schedule.


No. 2 - Allow yourself cheat meals
Sticking with the idea that keeping weight off needs to be a lifestyle change, it's important that you don't deprive yourself of everything you like. Eating healthily doesn't mean you can't ever have foods like burgers, ice cream and pizza. In fact, when most people start a diet like that, they fall off the wagon within the first month, overcompensate by eating a ton of the foods they tried to cut out of their diet, and end up gaining weight. The solution is to allow yourself to eat "cheat meals" as 10% of your meals. If you eat every two to three hours, you'll eat around six meals per day. Six meals per day times seven days per week is 42 meals per week. This means that about four of your meals throughout the week can be cheat meals. The key to a successful cheat meal is to eat something you enjoy, but do not overeat. In other words, have a couple slices of pizza, but don't eat the whole pie.


No. 1 - Surround yourself with healthy people
Your behavior inevitably mirrors that of the people you spend the most time with. If you surround yourself with heavy drinkers, you're likely to end up drinking more. If you surround yourself with junk food enthusiasts, you're likely to end up eating more junk food. On the contrary, if you surround yourself with people who play flag football more than they watch football on TV, you're likely to be more active. In a nutshell, if you spend the majority of your time with people who are active and health-conscious, you're more likely to be so yourself.

Ever heard of Pai You Guo? Apparently it is a weight loss supplement from China that, even though recalled in the US, continues to be sold here. It contains sibutramine (Meridia), which was the subject of a recent blog and was pulled from the market due to cardiotoxicity. Looks like it has also been linked to cancer.

A potentially hazardous weight loss supplement that has been recalled in the United States continues to be sold and consumed in significant amounts, a new study suggests.
The study found that women are taking Pai You Guo, a weight loss supplement from China that has been found to contain banned pharmaceutical substances, including a drug linked to cancer.

In 2009, the Food and Drug Administration (FDA) announced a voluntary recall of Pai You Guo and issued alerts about the supplement's potential health risks.
The new study found nearly a quarter of Brazilian-born women surveyed in Boston use Pai You Guo. No one who took the supplement was aware of the recall, and most experienced side effects.
The findings demonstrate a much larger problem — that regulation of dietary supplements in the United States is too lax, said study researcher Dr. Pieter Cohen, an assistant professor of medicine at Harvard Medical School and a general internist at Cambridge Health Alliance.
"Even when the FDA clearly determines that there's a hazardous product," the agency is incapable of effectively removing it from the market, Cohen said.
The doses of banned drugs in the supplements "convinces us that this was no error" on the part of the manufacturer, Cohen said.
Because consumers, standing in a store aisle, can't be sure whether the supplements they buy have been recalled or contain dangerous substances, they should steer clear of them for now, Cohen said.
"I do not believe that it's safe to purchase weight loss supplements in the United States at this time," Cohen said.
Adulterated pills?The FDA regulates dietary supplements, but the agency can only step in after the supplements come to market and recall ones they find to be hazardous. Since 2005, the FDA has identified more the 300 weight loss supplements that contain banned pharmaceuticals.
That's likely only a small fraction of the actual number of adulterated supplements, Cohen said.
Cohen and colleagues surveyed more than 500 women in the Boston area originally from Brazil. Earlier research identified a particularly high use of diet pills among Brazilian-born women.
Twenty-three percent reported using Pai You Guo, in either capsule or pill form. Eighty-five percent reported at least one side effect, including dry mouth, anxiety and insomnia. About 66 percent of users purchased Pai You Guo from acquaintances, 30 percent from stores and 9 percent over the Internet. Most participants bought the supplement after the recall.
People take Pai You Guo thinking they're using a natural weight loss remedy. "Instead, they're consuming banned pharmaceutical product," Cohen said.
Pai You Guo has been found to contain sibutramine, a weight loss drug that was recalled in 2010 due concerns it might increase the risk of heart attack and stroke. Some capsules and teas containing Pai You Guo have been found to contain doses of sibutramine six times higher than the doses found in prescription pills, Cohen said, and the substance is not even listed on the product label.
This problem likely extends beyond the Boston area, Cohen said. Pai You Guo has been reportedly sold in California, Florida and Washington state. "We're talking about a national problem," Cohen said.
Others say the problem is likely not common in the general population. It is likely confined to certain pockets of the population, said Dr. Bimal Ashar, an associate professor of medicine at John Hopkins School of Medicine in Baltimore.
Rotten tomatoes?Congress should re-write the rules that govern dietary supplements, Cohen said. The FDA should be given the authority to approve dietary supplements before they go to market, Cohen said, and should ensure the drugs are safe and really contain what's on the label, he said.
Right now, "supplements are regulated by the FDA as if they are food," Cohen said. But unlike, say, a spoiled tomato, "the consumers has absolutely no ability when they're popping a pill to know if it's spoiled or rotten or not," Cohen said.
Ashar agreed regulation of weight loss supplements should be enhanced. And the FDA, he said, should increase awareness about the way these products are regulated.
"I don't think the public really knows that this stuff is not regulated," before it comes to market, Ashar said. "They think ok, this is in a bottle…it must be pure, it must be fine to take."
"There needs to be a very, very highly publicized campaign about dietary supplement," in which the FDA says what it can do and will do to regulate these substances, Ashar said. However, even if better regulations are put in place, it may be tough to prevent the sale of these weight loss supplements if most are acquired through acquaintances, Ashar said. The FDA would not have the resources to track down such sales the way law enforcement tracks down the sales of narcotics, making it practically impossible, Ashar said.

LOS ANGELES – Blame the potato chip. It's the biggest demon behind that pound-a-year weight creep that plagues many of us, a major diet study found. Bigger than soda, candy and ice cream. And the reason is partly that old advertising cliché: You can't eat just one.
"They're very tasty and they have a very good texture. People generally don't take one or two chips. They have a whole bag," said obesity expert Dr. F. Xavier Pi-Sunyer of the St. Luke's-Roosevelt Hospital Center in New York.  What we eat and how much of it we consume has far more impact than exercise and most other habits do on long-term weight gain, according to the study by Harvard University scientists. It's the most comprehensive look yet at the effect of individual foods and lifestyle choices like sleep time and quitting smoking.

The results are in Thursday's New England Journal of Medicine.Weight problems are epidemic. Two-thirds of American adults are overweight or obese. Childhood obesity has tripled in the past three decades. Pounds often are packed on gradually over decades, and many people struggle to limit weight gain without realizing what's causing it.The new study finds food choices are key. The message: Eat more fruits, vegetables, whole grains and nuts. Cut back on potatoes, red meat, sweets and soda."There is no magic bullet for weight control," said one study leader, Dr. Frank Hu. "Diet and exercise are important for preventing weight gain, but diet clearly plays a bigger role."Doctors analyzed changes in diet and lifestyle habits of 120,877 people from three long-running medical studies. All were health professionals and not obese at the start. Their weight was measured every four years for up to two decades, and they detailed their diet on questionnaires.On average, participants gained nearly 17 pounds over the 20-year period.For each four-year period, food choices contributed nearly 4 pounds. Exercise, for those who did it, cut less than 2 pounds.Potato chips were the biggest dietary offender. Each daily serving containing 1 ounce (about 15 chips and 160 calories) led to a 1.69-pound uptick over four years. That's compared to sweets and desserts, which added 0.41 pound.For starchy potatoes other than chips, the gain was 1.28 pounds. Within the spud group, french fries were worse for the waist than boiled, baked or mashed potatoes. That's because a serving of large fries contains between 500 to 600 calories compared with a serving of a large baked potato at 280 calories.Soda added a pound over four years. Eating more fruits and vegetables and other unprocessed foods led to less weight gain, probably because they are fiber-rich and make people feel fuller.For each four-year period, these factors had these effects on weight:

— An alcoholic drink a day, 0.41-pound increase.
— Watching an hour of TV a day, 0.31-pound increase.
— Recently quitting smoking, 5-pound increase.People who slept more or less than six to eight hours a night gained more weight.The study was funded by the National Institutes of Health and a foundation. Several researchers reported receiving fees from drug and nutrition companies."Humans naturally like fat and sweet," said Dr. David Heber, director of the UCLA Center for Human Nutrition, who had no role in the study. "That's why we always tell people to eat their fruits and vegetables."Pi-Sunyer, who also wasn't involved in the research, said the study gives useful advice."It's hard to lose weight once you gain it," he said. "Anything that will give people a clue about what might prevent weight gain if they follow through with it is helpful."
The federal government earlier this year issued new dietary guidelines advising people to eat smarter. This month, it ditched the food pyramid — the longtime symbol of healthy eating — in favor of a dinner plate divided into four sections containing fruits, vegetables, protein and grains.
