The following should be helpful for those patients who either have diabetes or are at risk of getting diabetes who are using steroid medications.
Please make an appointment if you are taking steroids and have any questions about your blood sugar.


CORTICOSTEROIDS
Hyperglycemia can occur with non-oral corticosteroids...topical, inhaled, or local injections. Hyperglycemia is less likely when corticosteroids are given by these routes instead of systemically...but can still occur in susceptible patients.  Topical steroids can cause hyperglycemia when used in chronic or high doses...especially for diffuse conditions such as psoriasis. We monitor blood glucose in patients using high-potency steroids...large steroid doses...or when treatment exceeds 6 months. We are  especially vigilant if patients are at risk for diabetes.  Inhaled steroids can increase the risk of hyperglycemia when used in high doses, such as fluticasone 1000 mcg/day or more. If hyperglycemia occurs, we try to lower the steroid dose and add a bronchodilator or other agent if needed. Steroid injections into joints can cause hyperglycemia for about 2 to 5 days in patients with diabetes...and longer for long-acting steroid formulations. Epidural injections can increase blood glucose up to 2 weeks.

When are routine mammograms necessary: Whom do you believe?

So, who do you believe about screening for BREAST CANCER?
 
The American College of OB-GYN recommends screening mammograms starting at age 40. In 2009, the US Government Preventive Services Task Force said NO to routine mammograms until age 50.
Our suggestion? Go with the American College of OB-GYNs.

Update on Glucosamine and Chondroitin

Glucosamine and chondroitin sulfate supplements are used to slow the progression of osteoarthritis — the deterioration of cartilage between joint bones — and to reduce the associated pain. A study sponsored by the National Institutes of Health (NIH) called the Glucosamine/Chondroitin Arthritis Intervention Trial (or GAIT study), showed that the combination of glucosamine and chondroitin sulfate seems to be effective in osteoarthritis patients with moderate to severe knee pain, although not those with mild pain. However, a 2007 review of several past, large-scale studies of chondroitin alone concluded that it did not provide a benefit.
A well-controlled study of glucosamine sulfate (1,500 mg per day) versus placebo for lower back pain caused by osteoarthritis showed that both glucosamine and placebo improved symptoms reported by patients. However, there was no significant difference in the level of improvement between the groups, suggesting that glucosamine is not truly effective for lower back pain (Wilkens, JAMA 2010).
A six-month study of chondroitin sulfate (800 mg once daily) versus placebo in patients with hand osteoarthritis showed a significantly greater decrease in hand pain and improvement in hand function among those taking chondroitin sulfate (Gabay, Arth & Rheum 2011). The duration of morning stiffness was also slightly reduced. These benefits were not noticeable until after 3 months of treatment. There was no significant difference in grip strength or the amount of analgesic used by patients. The chondroitin tested in the study was a tablet containing highly purified (95% pure) chondroitin of fish origin and is marketed as Chondrosulf (IBSA, Switzerland) in Europe where it is approved as a “Symptomatic Slow Acting Drug for Osteoarthritis” (IBSA website – accessed 9/21/2011).
Results may vary from patient to patient, but I can not recommend the use of glucosamine at this time. It is possible that chondroitin may be beneficial for hand pain, but be patient as it could take up to 3 months to work.
Please remember that OTC arthritis pills like Advil or Aleve will help pain but do NOTHING to reverse arthritis. These medications do have a lot of potentially severe side effects and you should discuss their use with your physician.

Study Launched to Improve Bone Density Without Medication

The truth about the new FDA warnings about Simvastatin

 

 

 

Many patients have been notified by their pharmacist or insurance company about using the cholesterol drug, simvastatin. Please note that it has been on the market since the late 1980’s and I have never seen a case of severe, irreversible muscle damage with this or any other statin medication. (Neither do these medications cause liver damage!)
The FDA has now limited its dosage to 40 mgs or less. Our take is that if you have been on the 80 mg dose for years without exhibiting any side effects, there is no reason to change. Please note that taking simvastatin with certain antibiotics like erythromycins or antifungals may increase the drug levels in your body. Even grapefruit juice can hake the same effect. Certain blood pressure medications may also elevate drug levels (amlodipine, diltiazem).
Please note that LIPITOR, which goes generic soon, also interacts with erythromycins, amlodipine diltiazem and others, including grapefruit, too. Gemfibrozil should be avoided with EVERY statin!
The “cleaner” drugs like pravastatin or Crestor will also show interactions, but with far fewer drugs and supplements.
Remember, that STATINS and only STATINS (no other lipid lowering prescriptions nor HERBALS) have been proven to decrease cardiovascular mortality over the past generation.

Top 10 Tips for Preventing Weight Gain

 

This article was recently seen on the internet and I thought it was appropriate since it is basically our philosophy: Some tips are obvious, but others like #9,8,7,5 and 1 are also very important.

Shedding some weight can be hard. For many men, keeping it off is even harder. Diets become stale and depriving. Training becomes overly laborious and boring. Follow these 10 tips to keep off the weight that you've worked so hard to lose.

No. 10 - Throw out junk food
No matter how disciplined you are, if junk food is there, you'll eventually eat it. All it takes is a long day at work or a rushed meal to push you toward grabbing the quick fix. Do a quick search through your house (cabinets, refrigerator, under your pillow, and so on) and get rid of unhealthy snack food. If your spouse, significant other or kids have junk in the house, throw their stuff out, too. They'll thank you later.


No. 9 - Stay hydrated
Consuming a couple cups of water before you eat will fill up your stomach and cause you to eat less. Research has shown that people using this strategy lose more weight than people who don't. If you don't like water, try adding a lemon or lime to it or try drinking a zero-calorie beverage with a little more flavor like green tea or POWERADE ZERO. The important thing is that you're staying hydrated and filling up your stomach with liquid while not taking in additional calories.


No. 8 - Get a dog

A big part of keeping weight off is maintaining an active lifestyle. It sounds ridiculous, but research has shown that people tend to become more active when they have a pet. Letting your dog take you for a walk at night will get you off the couch and keep your hand out of the junk food that you should have thrown out already.


No. 7 - Eat breakfast
Your metabolism plummets while you sleep; you go into a state of catabolism (read: breakdown). The best way to jump-start your metabolism and give your body the fuel it needs for the day to come is to eat a quality breakfast. At a bare minimum, your breakfast should have at least one serving of fruits and vegetables, a lean protein source and a glass of water. You can also add a quality whole-grain carbohydrate source, such as steel cut oats, to your breakfast.


No. 6 - Avoid sugars, pasta and white bread
When you take in a whole-grain carbohydrate source, it takes time for your body to break down the food. As a result, you receive a slow, consistent supply of food, similar to a time-release drug. On the other hand, foods like pasta, white bread or anything high in sugar are rapidly digested and give your body a quick supply of a ton of energy. Since your body doesn't need all of this energy at once, it defaults to storing it as fat. Overall, these foods have little nutritional value and generally lead to metabolic damage and fat storage.


No. 5 - Learn how to cook
Short-term diet interventions never work long-term. In order to keep weight off, you need to change your lifestyle to make it conducive to keeping weight off. This sounds simple, but for some men it can be a rough transition. If you associate healthy eating with bland foods, you probably won't continue to eat healthily for too long. Learning how to use spices and prepare foods in different ways that you enjoy will help ensure that you're eating the right foods for the rest of your life. You can start by perusing the grocery store for prepackaged spices like Cajun, Italian seasoning, Montreal steak and/or chicken, and seasoned salt. Eating healthily does not need to be boring.


No. 4 - Lift weights
Muscle has a significantly higher metabolic rate than bone and fat. Simply put, this means that it takes more energy (read: calories) to fuel muscle than it does other tissues within the body. By lifting weights and adding some muscle mass, you keep your metabolism and the number of calories you burn at rest high. This is one of the most overlooked aspects of a quality fat-loss program and is essential as part of a weight-loss maintenance program.


No. 3 - Eat every 2 to 3 hours
One of the best things you can do to keep weight off is to set up an eating schedule and stick to it. Eating regularly tells your body that you are going to supply it with a consistent supply of quality nutrients and that it doesn't need to hoard anything. It will also ensure that you don't binge eat because you're "starving." Figure out when you usually wake up in the morning and when you go to bed at night, then plan meals every two to three hours in between. Remember that keeping weight off is about keeping your metabolism high. Skipping meals lowers your metabolism, too, so stick to your schedule.


No. 2 - Allow yourself cheat meals
Sticking with the idea that keeping weight off needs to be a lifestyle change, it's important that you don't deprive yourself of everything you like. Eating healthily doesn't mean you can't ever have foods like burgers, ice cream and pizza. In fact, when most people start a diet like that, they fall off the wagon within the first month, overcompensate by eating a ton of the foods they tried to cut out of their diet, and end up gaining weight. The solution is to allow yourself to eat "cheat meals" as 10% of your meals. If you eat every two to three hours, you'll eat around six meals per day. Six meals per day times seven days per week is 42 meals per week. This means that about four of your meals throughout the week can be cheat meals. The key to a successful cheat meal is to eat something you enjoy, but do not overeat. In other words, have a couple slices of pizza, but don't eat the whole pie.


No. 1 - Surround yourself with healthy people
Your behavior inevitably mirrors that of the people you spend the most time with. If you surround yourself with heavy drinkers, you're likely to end up drinking more. If you surround yourself with junk food enthusiasts, you're likely to end up eating more junk food. On the contrary, if you surround yourself with people who play flag football more than they watch football on TV, you're likely to be more active. In a nutshell, if you spend the majority of your time with people who are active and health-conscious, you're more likely to be so yourself.

For years now, we have been touting the benefits of eating nuts. At first, we noticed it’s benefits for hypertension. (low salt please!) This is because nuts are naturally high in magnesium and potassium which are known to help lower blood pressure. Now, let’s see what they can do for diabetes!
Sure, nuts are high in fat, but the GOOD FAT!

Eating Nuts Daily Could Help Control Type 2 Diabetes and Prevent Complications, Study Suggests

ScienceDaily (July 12, 2011) — Eating nuts every day could help control Type 2 diabetes and prevent its complications, according to new research from St. Michael's Hospital and the University of Toronto.

In the research, published online by the journal Diabetes Care, a team of researchers led by Dr. David Jenkins (University of Toronto Department of Nutritional Sciences; St. Michael's Hospital Risk Factor Modification Centre) reports that consuming two ounces of nuts daily as a replacement for carbohydrates proved effective at glycemic and serum lipid control for people with Type 2 diabetes.
"Mixed, unsalted, raw, or dry-roasted nuts have benefits for both blood glucose control and blood lipids and may be used as part of a strategy to improve diabetes control without weight gain," said Dr. Jenkins, who also has appointments with St. Michael's Division of Endocrinology and Metabolism and the U of T's Department of Medicine. He also serves as Canada Research Chair in Nutrition and Metabolism.
Jenkins and his colleagues provided three different diet supplements to subjects with Type 2 diabetes. One group was given muffins, one was provided with a mixture of nuts including raw almonds, pistachios, walnuts, pecans, hazelnuts, peanuts, cashews, and macadamias, and one group was given a mixture of muffins and nuts.
Subjects receiving the nut-only supplement reported the greatest improvement in blood glucose control using the glycosylated hemoglobin (HbA1c) test. The nut diet subjects also experienced a reduction in low-density lipoprotein cholesterol (known as LDL, or "bad cholesterol"). The subjects provided the muffin supplement or mixed muffin-and-nut supplement experienced no significant improvement in gylcemic control but those receiving the muffin-nut mixture also significantly lowered their serum LDL levels.
"Those receiving the full dose of nuts reduced their HbA1c [the
long-term marker of glycemic control] by two-thirds of what the U.S. Food and Drug Administration recognizes as being clinically meaningful for therapeutic agents. Furthermore, neither in the current study nor in previous reports has nut consumption been associated with weight gain. If anything, nuts appear to be well suited as part of weight-reducing diets," Dr. Jenkins said.
"The study indicates that nuts can provide a specific food option for people with Type 2 diabetes wishing to reduce their carbohydrate intake."

Why do we have an FDA?

 

Ever heard of Pai You Guo? Apparently it is a weight loss supplement from China that, even though recalled in the US, continues to be sold here. It contains sibutramine (Meridia), which was the subject of a recent blog and was pulled from the market due to cardiotoxicity. Looks like it has also been linked to cancer.

A potentially hazardous weight loss supplement that has been recalled in the United States continues to be sold and consumed in significant amounts, a new study suggests.
The study found that women are taking Pai You Guo, a weight loss supplement from China that has been found to contain banned pharmaceutical substances, including a drug linked to cancer.

In 2009, the Food and Drug Administration (FDA) announced a voluntary recall of Pai You Guo and issued alerts about the supplement's potential health risks.
The new study found nearly a quarter of Brazilian-born women surveyed in Boston use Pai You Guo. No one who took the supplement was aware of the recall, and most experienced side effects.
The findings demonstrate a much larger problem — that regulation of dietary supplements in the United States is too lax, said study researcher Dr. Pieter Cohen, an assistant professor of medicine at Harvard Medical School and a general internist at Cambridge Health Alliance.
"Even when the FDA clearly determines that there's a hazardous product," the agency is incapable of effectively removing it from the market, Cohen said.
The doses of banned drugs in the supplements "convinces us that this was no error" on the part of the manufacturer, Cohen said.
Because consumers, standing in a store aisle, can't be sure whether the supplements they buy have been recalled or contain dangerous substances, they should steer clear of them for now, Cohen said.
"I do not believe that it's safe to purchase weight loss supplements in the United States at this time," Cohen said.
Adulterated pills?The FDA regulates dietary supplements, but the agency can only step in after the supplements come to market and recall ones they find to be hazardous. Since 2005, the FDA has identified more the 300 weight loss supplements that contain banned pharmaceuticals.
That's likely only a small fraction of the actual number of adulterated supplements, Cohen said.
Cohen and colleagues surveyed more than 500 women in the Boston area originally from Brazil. Earlier research identified a particularly high use of diet pills among Brazilian-born women.
Twenty-three percent reported using Pai You Guo, in either capsule or pill form. Eighty-five percent reported at least one side effect, including dry mouth, anxiety and insomnia. About 66 percent of users purchased Pai You Guo from acquaintances, 30 percent from stores and 9 percent over the Internet. Most participants bought the supplement after the recall.
People take Pai You Guo thinking they're using a natural weight loss remedy. "Instead, they're consuming banned pharmaceutical product," Cohen said.
Pai You Guo has been found to contain sibutramine, a weight loss drug that was recalled in 2010 due concerns it might increase the risk of heart attack and stroke. Some capsules and teas containing Pai You Guo have been found to contain doses of sibutramine six times higher than the doses found in prescription pills, Cohen said, and the substance is not even listed on the product label.
This problem likely extends beyond the Boston area, Cohen said. Pai You Guo has been reportedly sold in California, Florida and Washington state. "We're talking about a national problem," Cohen said.
Others say the problem is likely not common in the general population. It is likely confined to certain pockets of the population, said Dr. Bimal Ashar, an associate professor of medicine at John Hopkins School of Medicine in Baltimore.
Rotten tomatoes?Congress should re-write the rules that govern dietary supplements, Cohen said. The FDA should be given the authority to approve dietary supplements before they go to market, Cohen said, and should ensure the drugs are safe and really contain what's on the label, he said.
Right now, "supplements are regulated by the FDA as if they are food," Cohen said. But unlike, say, a spoiled tomato, "the consumers has absolutely no ability when they're popping a pill to know if it's spoiled or rotten or not," Cohen said.
Ashar agreed regulation of weight loss supplements should be enhanced. And the FDA, he said, should increase awareness about the way these products are regulated.
"I don't think the public really knows that this stuff is not regulated," before it comes to market, Ashar said. "They think ok, this is in a bottle…it must be pure, it must be fine to take."
"There needs to be a very, very highly publicized campaign about dietary supplement," in which the FDA says what it can do and will do to regulate these substances, Ashar said. However, even if better regulations are put in place, it may be tough to prevent the sale of these weight loss supplements if most are acquired through acquaintances, Ashar said. The FDA would not have the resources to track down such sales the way law enforcement tracks down the sales of narcotics, making it practically impossible, Ashar said.

Antioxidant spices reduce negative effects of fight-fat meal

Eating a diet rich in spices, like turmeric and cinnamon, reduces the body's negative responses to eating high-fat meals, according to Penn State researchers.
"Normally, when you eat a high-fat meal, you end up with high levels of triglycerides, a type of fat, in your blood," said Sheila West, associate professor of biobehavioral health, Penn State, who led the study. "If this happens too frequently, or if triglyceride levels are raised too much, your risk of heart disease is increased. We found that adding spices to a high-fat meal reduced triglyceride response by about 30 percent, compared to a similar meal with no spices added."
West and her colleagues prepared meals on two separate days for six men between the ages of 30 and 65 who were overweight, but otherwise healthy. The researchers added two tablespoons of culinary spices to each serving of the test meal, which consisted of chicken curry, Italian herb bread, and a cinnamon biscuit. The control meal was identical, except that spices were not included. The team drew blood from the participants every 30 minutes for three hours. They reported their findings in the current issue of the Journal of Nutrition.
"In the spiced meal, we used rosemary, oregano, cinnamon, turmeric, black pepper, cloves, garlic powder and paprika," said Ann Skulas-Ray, postdoctoral fellow. "We selected these spices because they had potent antioxidant activity previously under controlled conditions in the lab."
When the meal contained a blend of antioxidant spices, antioxidant activity in the blood was increased by 13 percent and insulin response decreased by about 20 percent.
According to West, many scientists think that oxidative stress contributes to heart disease, arthritis and diabetes. "Antioxidants, like spices, may be important in reducing oxidative stress and thus reducing the risk of chronic disease," she said, adding that the spice dose they used provided the equivalent amount of antioxidants contained in 5 ounces of red wine or 1.4 ounces of dark chocolate.
Skulas-Ray noted that adding two tablespoons of spices to meals did not cause stomach upset in the participants. "They enjoyed the food and had no gastrointestinal problems," she said. But, she added, "The participants were notified ahead of time that they would be eating highly spiced foods and they were willing to do so."
In the future, West plans to investigate whether she can get the same results by adding smaller doses of spices to meals.


Other Penn State researchers on the paper include Ann Skulas-Ray, graduate student; Penny Kris-Etherton, Distinguished Professor of Nutrition; Danette Teeter, former research assistant; and John Vanden Heuvel, professor of veterinary science. Chung-Yen (Oliver) Chen, scientist, Tufts University, also was involved in the study.

Bang for your buck?

 



"CocoaVia and Cirku are new supplements used for high blood pressure and cardiovascular health. They are flavored powders that can be added to a beverage. Each packet contains a cocoa extract providing 350 mg of cocoa flavanols. Research on cocoa flavanols for heart disease is promising. Some research shows that consuming 213-500 mg/day of cocoa flavanols can decrease systolic blood pressure by about 5 mmHg and diastolic by about 3 mmHg. Increasing cocoa intake is also associated with reduced risk of cardiovascular and all-cause mortality.”

From a physician's point of view:
Sounds promising plus natural: Only problem is, most patients need at least a 10-20 point drop in their systolic (top number) to reach goal, And this is for MILD hypertension. Moderate HBP would not only require a 20+ point drop in pressure but usually requires two or more medications. I don’t know how much these supplements cost, but generic lisinopril cost $4/month even without insurance, has been on the market for 25 years and has a great safety profile.

Here we go again. Who knows? There have been several studies showing that high doses of calcium supplements can increase the risk of heart disease, especially if taken on an empty stomach.
Our recommendations: Try to take 500 mg/day of a supplement and get the rest from vitamin enriched foods and exercise.
Genetics and life style play a large role and it seems to be much more important to get enough VITAMIN D and sunshine in your life as well as regular TOTAL BODY WEIGHT RESISTANCE EXERCISES.
Sure, prescriptions can help, but I have seen so many failures that medications should only play a part of  your treatment. Of course, they are expensive and now come loaded with increased long side effects after you have been on them for more then 5 years.

New Guidelines on Sunscreens - Starting Summer 2012

 

 

 

- Please note the 2 types of sun rays that should be blocked: UVA and UVB.
- More importantly, please be aware of products’ claims of “waterproof”. - Sunscreens have to be reapplied every 2 hours to be effective.


New rules for sunscreens will give consumers much-needed help picking an appropriate sunscreen...starting next summer.    • FDA will allow sunscreens to be labeled "broad spectrum" ONLY if they provide adequate protection against both UVA and UVB rays.    This is important.    UVB causes visible sunburn...UVA causes more underlying skin damage. Current SPF ratings apply only to UVB protection.    • Sunscreens that contain only UVB protection reduce the obvious signs of burning so people THINK they're protected...but the lack of UVA protection allows damage below the surface.    • New products will also no longer claim to be "waterproof" or "sweat proof"...all sunscreens eventually wash off or become less effective when they get wet.    Instead, sunscreens will state if they are "water-RESISTANT" for 40 or 80 minutes. I recommend reapplying sunscreen at least every 2 hours and after swimming or sweating.    For now, I tell patients to look for sunscreens that contain ingredients that protect against UVA rays, such as avobenzone, zinc oxide, and/or titanium dioxide.
   Limit sunscreen use in infants less than 6 months. Advise applying it to exposed skin only if adequate shade and clothing are not possible...and wash it off afterwards to minimize exposure.

Chantix Controversy?

 

 

 

I can think of no greater risk to health than smoking. Recently the FDA noted an increased risk of heart disease with CHANTIX-the gold standard for quitting. Don’t believe it. If there is a risk, it is miniscule.
The FDA has also warned about anxiety and depression with the use of CHANTIX. However most symptoms, in retrospect are due to nicotine withdrawal.
And why quit smoking?

Protein: Ranked From Best to Worst

 

Here is a summary from a great article in Men’s Journal about dietary protein. It was published in the August 11, 2011 edition and a copy can be found in our office.

Best to Worst:

1. EGGS: For years, I’ve been telling our patients that eggs are not associated with elevated cholesterol or heart disease. The best part: The yolk. Incredible protein and only 70 calories. One note: I would stick to hard boiled eggs. When the yolk is broken (like with an omelet) this can lead to oxidation and potential harmful side effects)
2.POULTRY: WHITE meat please and NO SKIN. Please bake and remember no breading or frying!
3.FISH: Great source of protein and omega-3 fatty acids. Please remember that most farm fist may be contaminated with mercury and PCB’s, so choose wild whenever possible. NOTE to pregnant women. Watch out especially for swordfish which is very high in mercury.
4. DAIRY: Casein helps to prevent muscle breakdown. Aim for lower fat cheeses as regular cheeses are too high in fat. AND watch portion sizes!
5. BEANS: Not only a great source of protein but also FIBER. Also, not a bad choice for vegetarians. There is a little fat but it is polyunsaturated.
6. SOY: Low fat-high protein. Edamame is great, but who likes TOFU?
7. RED MEAT: A great source of protein and vitamins but also fat. It really helps to control appetite, too. Use in moderation.
8. NUTS: These do not contain all the protein the body needs, so it is OK as a supplement to your diet. Please limit quantities because they can be very fattening! The best: WALNUTS. Nuts are also
high in potassium and magnesium and may help to lower blood pressure. Please stay away front honey roasted and highly salted nuts. READ THE LABELS!
9.PROTEIN POWDER AND BARS: We have always been an advocate of using high protein, low carb supplements as part of mid-day or evening snacks, especially on the run. Just make sure you are using whey protein (casein and soy won’t work as well) and don’t be afraid to ask the clerk at the store for a taste test first. Our patients usually receive a discount at GNC and MAXWELLNESS if they mention my name.

You might be surprised, but a study conducted by the University of Texas found that low fat chocolate milk drinkers not only had better workouts but also had decreased muscle recovery time! They compared chocolate milk to carbohydrate and protein drinks. So, you might want to remember this the next time you see a fancy national TV ad with sports’ superstars hawking expensive sports drinks!

Vitamin B Supplements: Avoid Them!!!

 

 

 

There are eight B vitamins generally known as thiamin (B-1), riboflavin (B-2), niacin (B-3), pantothenic acid (B-5), pyridoxine (B-6), biotin (B-7), folic acid (folate) (B-9), and cyanocobalamin (B-12). Like most vitamins, B vitamins are essential — your body needs them but cannot make them; you must get them from your diet or from supplements.
Each B vitamin is associated with a variety of functions, many of which relate to turning food into energy and other needed substances. Folate, for example, is important in reducing birth defects of the spinal cord and is, therefore, a critical ingredient in prenatal vitamins. But at a high dosage, folic acid may increase the risk of certain cancers. Niacin, when taken in very high doses, can improve cholesterol levels but can also cause side-effects and toxicity. Several (B-6, B-12, and folate) have been shown to reduce elevated homocysteine levels -- a risk factor for cardiovascular disease, although studies have not shown this combination to reduce cardiovascular risk itself.

Previous reviews by ConsumerLab.com have uncovered problems with the B vitamin content of several supplements. Neither the U.S. government nor any agency is responsible for routinely testing B vitamin products or other dietary supplements for their contents or quality.

5-Hour Energy (Berry) contained only 75.5% of its claimed folic acid, although it did contain its claimed amounts of niacin, B-6, and B-12. The amount of B-6 and B-12 in this product is many times higher than the RDAs for these ingredients, although not exceeding tolerable upper limits for those vitamins. For young adults, the RDA for B-6 is 1.3 mg and for B-12 is 2.4 mcg, while the suggested serving (one bottle) provides 40 mg of B-6 (3,100% of the RDA) and 500 mcg of B-12 (20,800% of the RDA). ??5-Hour Energy lists among its ingredients an "energy blend" that includes caffeine. The label notes that the amount of caffeine is comparable to that in a cup of "leading premium coffee." ConsumerLab.com found the 2 fluid ounce bottle to contain 207 mg of caffeine. However, this is 15% higher than what you would get from a "short" cup (8 fluid ounces) of a premium coffee such as Starbucks, which Starbucks claims to have 180 mg of caffeine (Starbucks.com 2010).


 Stacker 2 6-Hour Power Shot contained just 40.4% of its claimed folic acid. It provided the same high levels of B-6 (40 mg) and B-12 (500 mcg) as 5-Hour Energy. Also, like 5-Hour Energy, 6-Hour Power Shot also lists among its ingredients an "energy blend" that includes caffeine. The label notes that the amount of caffeine is comparable to that in a cup of "brewed coffee". ConsumerLab.com found the 2 fluid ounce bottle to contain 156 mg of caffeine. However, this is 64% higher than what you would get from a cup (8 fluid ounces) of brewed coffee based on measurements by the USDA of coffee brewed at home and from fast food restaurants (USDA National Nutrient Database 2010)).

Unlike 5-Hour Energy which directs the user not to exceed 2 bottles per day, 6-Hour Power Shot only cautions not to exceed 1 shot every six hours -- which means taking as much as four bottles per day. Be aware that taking 3 or 4 bottles of 6-Hour Power can result in exceeding the Upper Tolerable Intake Levels for several B vitamins (niacin, vitamin B-6, and folic acid), creating a risk of toxicity. Intake exceeding 1,000 mg of folic acid, for example, may increase the risk of precancerous tumors becoming malignant: If 6-Hour Power contained the amount of folic acid that it claimed, 3 to 4 bottles would provide 1,200 mg to 1,600 mg of folic acid, exceeding the ULs. It would be prudent to limit intake of 6-Hour Power to under 2.5 bottles per day. B Complexes:?Supplements containing more than one B vitamin are widely sold. However, two common reasons for buying these complexes are not well supported by scientific evidence.

Our Recommendations:

   AVOID! AVOID AVOID!
   Try to avoid taking too much of any B vitamin. Check the ULs listed above and, if you need to take a product with vitamins exceeding the ULs, do so under a physician's supervision and with awareness of potential side effects. For example, a recent large study of diabetes patients with advanced nephropathy (kidney disease) given high-dose B vitamins (folic acid - 2.5 mg, B-6 - 25 mg, and B-12 - 1 mg daily) showed a worsening of kidney function and doubling of the risk of myocardial infarction (MI), stroke, and death in the B-vitamin group compared to the placebo group. The study was designed with the expectation that the vitamins would have the opposite effect because of the ability of B vitamins to lower homocysteine levels, which did decrease. (House, JAMA 2010)
  Be aware that certain B vitamins can interact with other drugs and supplements as noted above.

Researchers report that the more physically active you are, the less your blood pressure rises in response to a high-salt diet.  Investigators compared study participants' blood pressure on two 1-week diets, one with low sodium content, one with high.  The most physically active group were the least salt-sensitive, i.e., their average systolic blood pressure was least affected when they went on the high salt diet. The American Heart Association recommends consuming less than 1,500 mg/day of sodium.  If you are eating restaurant food and packaged food from the grocery store, you are getting more than a healthy amount of sodium even if you never use the salt shaker at home.  Add in more physical activity and keep your cardiologist (and body) happy.
