On February 28, 2013, the FDA issued a warning letter to Healthwest Minerals, Inc., dba Mt. Capra Products, Mt. Capra Whole Foods, and Mt. Capra Cheese, following a facility inspection which found the company’s products, including Coconut Dream Deep 30, Banana Smoothie Deep 30, Strawberry Splash! Deep 30, Double Bonded Protein, and CapraGreens products to be adulterated because they were were prepared, packed, or held under conditions that violate Current Good Manufacturing Practices for dietary supplements.

On March 21, 2013, the FDA warned consumers not to purchase or use three sexual enhancement dietary supplements, Rock-It Man, Libido Sexual Enhancer, and Stiff Days, because they were found to contain sildenafil (VIAGRA)or sildenafil analogues.
Sildenafil is the active ingredient in an FDA approved prescription drug for the treatment of erectile dysfunction. This drug can cause symptoms like headache and flushing, and can interact with medications containing nitrates such as nitroglycerine, resulting in dangerously low blood pressure.

On March 7, 2013, Lifestyle Evolution Inc. issued a recall of various NuGO nutrition bars because the bars may contain undeclared milk, a potential allergen.
People with a milk allergy should not consume these bars because they could cause an allergic reaction, which can be life-threatening

What is 'Osteogenic Loading?' - bioDensity training

bioDensity13

bioDensity training - osteogenic loading-  has been studied for the past 6 years. Osteogenic loading improves muscle and bone mass/strength using isometric, low impact exercises. The bioDensity machine allows us to actually measure these improvements.
Over 22,000 patients have been studied and the results are as follows:

1.After 3 weeks, we see, on average, a 45% improvement in force production.
2. After 6 weeks, that improves to 66%.
3. After 9 weeks we see, on average an 89% increase in bone and muscle mass.

(place Variance graph here)

Please note that we are able to compare your strength and progress with patients of all ages!

Weight loss statements from the New England Journal of Medicine (and Kaizen's 2 cents!)

weighloss2

(published online January 31, 2013)

1. Moderate lifestyle changes can be as effective as weight loss medications. (Easier said than done!)
2. Diets do help people lose weight, but attempting it and doing it are two different things. (Absolutely true)
3. Physical activity can have health benefits even in the absence of weight loss. (I have written about this many times in the past)
4. Providing actual meals or using meal replacements works better than general advice. (This is why we use a registered dietician)
5. Weight loss prescriptions and bariatric surgery can help with long term weight loss. (Qysmia has FDA approval for long term use, but only 305 of insurance companies will pay for it)
6. Small changes in diet/exercise will  not produce long term weight loss. (We don’t know why, but over 85% of people who lose weight will gain it all back no matter what kind of program they are on)
7. Realistic weight loss goals WILL NOT keep people motivated. (I don’t believe this statement-accountability will help keep people motivated. That’s why The Biggest Loser is so effective, until the people no longer have access to that kind of support)
8. Slow weight loss is not as good as rapid weight loss using very low calorie diets. (I don’t know where they got this from, but it is absolutely flawed. We know that fad diets do not work long term unless you plan to live on 500 calories the rest of your life. All rapid weight loss does is give you a quick fix, which is not only not sustainable, but also leads to decrease muscle mass and preservation of fat----“normal weight obesity”)
9. Sexual activity does not burn enough calories to lose weight. (Agreed)
10. Skipping breaking has no bearing on weight loss. (I have to disagree. Eating a nutritious breakfast after not eating for 8-12 hours is very beneficial and sets the tone for the rest of the day)
11. Adding fruits and vegetables do not help weight loss. (Yes a calorie is a calorie but when we recommend adding fruits and veggies, we are assuming that you are not eating chips, candy and soda!)

More FDA warnings from Kaizen Total Wellness

FDAsupplements

1.  On October 15, 2012, the FDA issued a warning letter to BioNeurix Corporation after statements found on the company’s website, http://www.pharma-tree.com, were found to promote the dietary supplements Blood Sugar 360, Amoryn, Seredyn, and Mellodyn as drugs.
PharmaTree Blood Sugar 360 contains chromium, alpha lipoic acid, Gymnema sylvestre, ginseng and banaba (Lagerstroemia speciosa). It was promoted to reduce blood sugar levels, regulate insulin sensitivity, and “decrease the chance of glucose-related nerve damage.” (This is so false that it doesn’t even deserve a comment!)
Amoryn contains St. John’s Wort (Hypericum perforatum), 5-HTP (5-hydroxytryptophan), Rhodiola rosa, vitamin B6, vitamin B12, folate, selenium, vitamin D3, zinc and vitamin C. It was promoted for the treatment of depression. Pure St. John’s Wort is probably effective but the rest is nonsense. Plus the best St. John’s Wort is made in Germany-where it is regulated.
Seredyn contains L-theanine, passion flower extract, valerian extract, niacinamide and magnesium taurinate. It was promoted for the treatment of insomnia and panic attacks. More lies.
Mellodyn contains melatonin, valerian root extract, chamomile, passion flower extract, and lemon balm. It was promoted for the treatment of insomnia. Ever smell Valerian?

2.  On December 7, 2012, the FDA issued a warning letter to Basic Organics, Inc. following a facility inspection which found the company’s Glucosamine Chondroitin and Omega 3 dietary supplements to be adulterated.
Do you know where your supplements come from?

3.  On December 19, 2012, P&J Trading issued a voluntary recall of SLIMDIA REVOLUTION after FDA testing found the dietary supplement contains undeclared sibutramine.
Sibutramine is a controlled substance which was removed from the prescription drug market in 2010 due to the potential for life-threatening side-effects and interactions with other medications. This substance can increase blood pressure and pulse rate, and may present a significant risk for individuals with a history of coronary artery disease, congestive heart failure and arrhythmias.
SLIMDIA REVOLUTION was distributed nationwide from March 2012 to December 2012.
The product also contains Hoodia gordonii, fenugreek seed, pysllium seed husk, dandelion leaf, fennel seed, hawthorn berries, ginkgo leaf and gotu kola. It was promoted for increased energy and weight loss. Once again, Meridia not only was ineffective but was taken off the market in 2010. Hoodia was also removed from the marketplace! Do you know what is really in your weight loss supplements?

4.  On December 18, 2012, Matrixx Initiatives issued a voluntary recall of a single lot of Zicam Extreme Congestion Relief nasal gel after routine testing confirmed a sample from this lot was contaminated with Burkholderia cepacia. The affected product is labeled with lot number 2J23 and an expiration date of September 2015. It was distributed by retailers nationwide. This deserves no further comment since it is well know that nasal zinc can induce the loss of smell and it can be permanent!

What you need to know about sugar: Link to informative article

HighFructose

A Kaizen Total Wellness blog follower sent the link to this article in Time Magazine. I could not have said it any better - fructose IS toxic!! Read the myths and go to link for more detailed information.

What You Need to Know About Sugar

By Robert H. Lustig, M.D., Dec. 27, 2012, Time Magazine

Read more: http://ideas.time.com/2012/12/27/what-you-need-to-know-about-sugar/#ixzz2GY9Odmvf
In 2009, an hour and a half lecture about sugar and obesity that Dr. Lustig gave to the public was posted to YouTube. Three million views later, the video is still going strong, and his theories about sugar’s toxic effects on the body are gaining traction. He still maintains that one particular form of sugar—fructose—is toxic in high dose. Yet there is still a lot of confusion about this dietary bogeyman. Here are five myths about sugar and some important distinctions about how our body processes its different forms.

Myth 1. A carbohydrate is a carbohydrate; they all have the same calories.

Myth 2. Fructose is turned into glucose in the body.

Myth 3. People can limit their sugar consumption without any difficulty.

Myth 4. High fructose corn syrup is worse than table sugar.

Myth 5. Fructose is natural — it’s found in fruit — so it couldn’t possibly be poisonous.

Our nutritionist can provide recipes and examples of ways to avoid fructose and high fructose corn syrup without feeling deprived of sweetness.

DASH Diet, hypertension and the Kaizen Total Wellness Center

DASH

Many people have heard about the DASH diet but few actually know what it is.

DASH stands for Dietary Approaches to Stop Hypertension. It emphasizes eating fruits, vegetables and low-fat dairy products. These foods are rich in calcium, potassium and magnesium which may help lower blood pressure.

Notice how this does not mention SALT. Salt is only a minor contributor to high blood pressure in most patients. The problem is that we eat too much of it. Please note that DASH also says nothing about Carbohydrates, but remember how fattening they are.

The sample DASH diet would include:

1.   6 servings of grains (preferably WHOLE GRAINS)

2.   4 servings of veggies

3.   4 servings of fruits

4.   2 servings of low fat or fat free dairy

5.   3-5 ounces of lean meats, poultry or fish

6.   1 tsp. oils (olive oil, please!)

7.   1/2 cup beans, 1/3 cup low salt nuts (avoid honey roasted, please)

8.   limit sweets, like frozen yogurt to 3 times/week.

For more information, and to tailor this to your individual needs, please make an appointment with our certified nutritionist, Patricia.

Kaizen Total Wellness' Power Plate client takes golf championship in Scotland!

Palm Aire's Jim Pfrogner returned from Dundee, Scotland with "yet another" golf title....A world championship! Pfrogner, a single-digit handicapper won the 4-day World Amateur Left-handed Master Division Championship in Scotland by 18 strokes.

" I have been training with the Power Plate for about 10 weeks. I have an arthritic problem in my lower legs and it has affected my quality of life. Since I've been training I have noticed positive results. This method of developing fitness is 30 minutes of intense training with a personal trainer twice a week. Additionally, a physical therapist works with me weekly and monitors my progress as well as updates my workouts. This is as good as it gets!

I'm becoming so fit and my arthritis problem is not nearly as painful as when I first started. I totally endorse Kaizen Total Wellness Program and of course the dynamic Power Plate. And another positive, my golf game has also improved! Thank you."   - Jim Pfrogner

Congratulations Jim, from Kaizen Total Wellness!! We are extremely proud!

A Kaizen Total Wellness Health Alert: Sugary beverages linked to heart disease, obesity, liver disease in recent study

Recently we warned of diabetes, obesity and even liver disease for those who consume sugar sweetened drinks (Sodas and Fruit Drinks). Here is a study from the journal CIRCULATION (March 12,2012) that now links sugar to HEART DISEASE, ABNORMAL LIPIDS, INFLAMMATION AND DECREASED LEPTIN LEVELS (the hormone that makes you feel full and satisfied).
They found a 20% higher relative risk of these disorders in HEART DISEASE in men between 40-75 years old who drank on average 6.5 drinks/week.
Please note that there were NO ADVERSE events noted with ARTIFICIAL SWEETENERS.

Power Plate Training Video

Power plate training at Kaizen Total Wellness Gym is always one on one.

Maintaining a Healthy Lifestyle: Laughter is Important!

An elderly man was stopped by the police around 2 am and was asked where he was going at this time of night.

The man replied, " I am on my way to a lecture about alcohol abuse and the effects it has on the human body, as well as smoking and staying out late."

The officer then asked, "Really? Who is giving that lecture at this time of night?"

The man replied, "That would be my wife."

This man is obviously not a member of Kaizen Total Wellness, where he would have availed himself of the services of a nutritionist, certified fitness trainers,  or a massage therapist. Probably why he was out all night getting into trouble, no doubt!

We have a professional team of wellness consultants that also includes our physical therapist.
Our gym contains the exclusive combination of fitness/rehabilitative equipment: bioDensity and Power Plate machines.
Medicare and insurance may be accepted for certain services.
Programs and services are available to everyone in the community.

Celebrities using Power Plate

Clint Eastwood
Actor and Director

Power Plate is a fast and effective strength workout and I like it before a round of golf because it helps me be loose and flexible. Many things come and go in this industry but the Power Plate machine is here to stay. It is a fast and effective way to stay in shape.”

Sting
Lead Singer, Sting and the Police

“The band must be in peak physical form to withstand the demands of daily travel and nightly performances. The Power Plate is that key component in helping us feel and look our best. Taking it on the road made a big difference.”

Roberta Flack
Singer

“The Power Plate has been instrumental in keeping me energized and healthy when I’m on tour. With a grand piano on stage and a Power Plate in my suite, I am ready to travel and perform anywhere in the world. I absolutely love it.”

Serena Williams
U.S., Australian and Wimbledon tennis champion

“Off court training is as important to me as on court. With the Power Plate, I’m able to accelerate my off court training and maximize the benefits.”

And a comment about vibration training:
Vice Admiral Regina M Benjamin MD MBA
US Surgeon General

From the Office of the US Surgeon General:?“Weight-bearing, strength, and balance-training exercises are also an important part of any osteoporosis prevention and treatment program, regardless of age. They can help increase or preserve bone mass and may also help reduce the risk of falling. All types of physical activity can contribute to bone health. Activities that are weight bearing or involve impact are most useful for increasing or maintaining bone mass. Activities that are not weight bearing or are low impact may help improve balance and coordination and maintain muscle mass, which can help prevent falls. For those who cannot engage in regular physical activity due to disability, mechanical stimulation of the skeleton might prove beneficial. Recent, small studies found that use of vibrating platforms increased BMD and slowed bone loss (Rubin 2004 et al., Verschueren et al. 2004, Ward et al. 2004). This may provide another way to reduce fracture risk both in the elderly and in younger individuals with disabling conditions that limit their ability to exercise.”

Study Launched to Improve Bone Density Without Medication

We have been using this technology at Kaizen Total Wellness® for the past 1 1/2 years with a great deal of success! We are currently the only facility in Florida offering this combination of technologies. If you have been diagnosed with osteopenia or osteoporosis, this may be used by itself or in conjunction with prescription medications.

BStrong4Life™,  a Chicago-based wellness company has launched a 6-month pilot study to determine the effectiveness of their system on improving bone mineral density without medication. The system is a combination of two technologies: Power Plate, and bioDensity,  as well as a proprietary training system that integrates the two.
Power Plate equipment works on the principles of whole-body vibration. This proprietary system uses warm-up procedures to prepare the user for each bioDensity session and to optimize performance. The Power Plate equipment vibrates almost imperceptibly, causing whole body vibrations that create instability and trigger reflexive contractions in the muscles, as frequently as 25 to 50 times per second.
The equipment also oscillates, so the muscles must contract in multiple dimensions. This technique stimulates the fast-twitch muscle fibers, which can lead to increased muscle strength, endurance and overall agility. The user stands on the equipment while performing either static or dynamic exercises.
The user creates as much muscular force as possible while performing four comprehensive static isometric muscle contractions. The equipment uses a patented load cell/software interface that precisely measures, tabulates and records muscular force output at 30 times per second. All physical efforts are recorded, and compared and contrasted over time. Each user receives a Performance Report at the conclusion of each training session.

Only 15 minutes a day can lengthen your life!

At both Kaizen Total Wellness and in my medical practice we try to get all of our patients to lead a more active lifestyle. Whereas 15 minutes of moderate exercise increasing life span by 3 years doesn’t seem that impressive, we find that the quality of life is dramatically improved both physically and mentally. We like our patients to try to do aerobic activities (walking, biking, swimming, etc.) for at least 30 minutes everyday and weight resistance 2-3 times a week. Our low impact weight resistance programs are safe for seniors and 15 minutes twice a week with our fitness trainers is equivalent to spending 1 hour three times a week in a conventional gym.
Call the office for a free demo of our Power Plate and bioDensity machines. This may be covered by insurance.

Fifteen Minutes of Moderate Daily Exercise Lengthens Life, Taiwanese Study Finds

ScienceDaily (Aug. 17, 2011) — Taiwanese who exercise for 15 minutes a day, or 92 minutes per week, extended their expected lifespan by three years compared to people who are inactive, according to a study published in The Lancet"Exercising at very light levels reduced deaths from any cause by 14 percent," said study senior author Xifeng Wu, M.D., Ph.D., professor and chair of The University of Texas MD Anderson Cancer Center Department of Epidemiology. "The benefits of exercise appear to be significant even without reaching the recommended 150 minutes per week based on results of previous research."
Lead author Chi-Pang Wen, M.D., of the National Health Research Institutes of Taiwan, and colleagues also found that a person's risk of death from any cause decreased by 4 percent for every additional 15 minutes of exercise up to 100 minutes a day over the course of the study. Those exercising for 30 minutes daily added about four years to life expectancy.
"These benefits were applicable to all age groups, both sexes and those with cardiovascular disease risk," the authors note.
If inactive people in Taiwan were to do low-volume daily exercise, one in six deaths could be postponed by their reduced risk of dying, the authors report. It would be an estimated reduction in mortality similar to that from a successful tobacco control program.
The prospective observational study involved 416,175 Taiwanese who participated in a standard medical screening program run by MJ Health Management Institution between 1996 and 2008. Participants were followed for an average of eight years.
For the exercise study, participants completed a questionnaire covering their medical history and lifestyle information. They characterized their weekly physical activity for the previous month by intensity -- light (walking), moderate (brisk walking), vigorous (jogging) or high vigorous (running) -- and time.
To account for occupational effects, participants also characterized their physical activity at work, ranging from sedentary to hard physical labor.
Those who reported less than one hour a week of leisure time physical activity were classified as inactive -- 54 percent of all participants. Others were classified as low, medium, high or very high based on the duration and intensity of their exercise. Researchers calculated mortality risk and life expectancy for each group.
Thirteen other variables were analyzed: age, sex, education level, physical labor at work, smoking, alcohol use, fasting blood sugar, systolic blood pressure, total cholesterol, body mass index, diabetes, hypertension and history of cancer.
Those who engaged in low-volume exercise had lower death rates than inactive people regardless of age, gender, health status, tobacco use, alcohol consumption or cardiovascular disease risk.
The researchers note that the World Health Organization and the U.S. Centers for Disease Control and Prevention recommend at least 150 minutes of moderate-intensity exercise per week. A third of U.S. adults meet that guideline; about 20 percent of adults in China, Japan or Taiwan meet it.
"A recommendation of 15 minutes of daily exercise should be promoted to East Asian populations," the authors note.
The study's findings of reduced mortality through even moderately intense exercise are likely to hold true for other populations, Wu said, even though the total amount of time spent or workout intensity required for a health benefit might differ. "These findings can stimulate people to exercise as much as they can and to not be frustrated that they can't reach the 30 minute per day guideline."
This is the first collaboration between Wu, Wen and the MJ Health Group, a major health screening company with more than 600,000 participants in its health database. They have formed the Asian Health Screening Cohort to conduct major research projects. Wu provides scientific expertise with Wen, who also is based at China Medical University Hospital, while MJ Health Group contributes patient epidemiological and clinical data as well as a biobank of tissue samples.
Two other ongoing collaborative projects include development of a liver cancer risk prediction model and a study of telomere length, genetic variation and cancer risk. The second project is funded by an MD Anderson Sister Institute Network Fund Grant. MD Anderson and China Medical University Hospital have a sister institution agreement.
The exercise project was funded by the Taiwan Department of Health Clinical Trial and Research Center of Excellence and the Taiwan National Health Research Institutes.
Co-authors with Wen and Wu are: co-lead author Jackson Pui Man Wai, Ph.D., of the Institute of Sport Science, National Taiwan Sport University; Min Kuang Tsai, Yi Chen Yang and Hui Ting Chan of the Institute of Population Science, Taiwan National Health Research Institutes; Tsai and Yang also are with the China Medical University Hospital; Ting Yuan David Cheng, University of Washington Department of Epidemiology; Meng-Chih Lee, M.D., Institute of Medicine and Department of Family and Community Medicine, Chung Shan Medical University and Hospital; Chwen Keng Tsao of MJ Health Management Institution; and Shan Pou Tsai, Ph.D., The University of Texas School of Public Health at Houston.

Why do we have an FDA?

Ever heard of Pai You Guo? Apparently it is a weight loss supplement from China that, even though recalled in the US, continues to be sold here. It contains sibutramine (Meridia), which was the subject of a recent blog and was pulled from the market due to cardiotoxicity. Looks like it has also been linked to cancer.
At Kaizen Total Wellness, we recommend weight loss the the old fashioned way: healthy eating and exercise. We do not sell any products, so we feel we are able to comment on supplements objectively.
For more information, please call 941-556-7788.Dieters May Unknowingly Buy Dangerous Weight Loss Supplements.

MyHealthNewsDaily
A potentially hazardous weight loss supplement that has been recalled in the United States continues to be sold and consumed in significant amounts, a new study suggests.
The study found that women are taking Pai You Guo, a weight loss supplement from China that has been found to contain banned pharmaceutical substances, including a drug linked to cancer.

In 2009, the Food and Drug Administration (FDA) announced a voluntary recall of Pai You Guo and issued alerts about the supplement's potential health risks.
The new study found nearly a quarter of Brazilian-born women surveyed in Boston use Pai You Guo. No one who took the supplement was aware of the recall, and most experienced side effects.
The findings demonstrate a much larger problem — that regulation of dietary supplements in the United States is too lax, said study researcher Dr. Pieter Cohen, an assistant professor of medicine at Harvard Medical School and a general internist at Cambridge Health Alliance.
"Even when the FDA clearly determines that there's a hazardous product," the agency is incapable of effectively removing it from the market, Cohen said.
The doses of banned drugs in the supplements "convinces us that this was no error" on the part of the manufacturer, Cohen said.
Because consumers, standing in a store aisle, can't be sure whether the supplements they buy have been recalled or contain dangerous substances, they should steer clear of them for now, Cohen said.
"I do not believe that it's safe to purchase weight loss supplements in the United States at this time," Cohen said.
Adulterated pills? The FDA regulates dietary supplements, but the agency can only step in after the supplements come to market and recall ones they find to be hazardous. Since 2005, the FDA has identified more the 300 weight loss supplements that contain banned pharmaceuticals.
That's likely only a small fraction of the actual number of adulterated supplements, Cohen said.
Cohen and colleagues surveyed more than 500 women in the Boston area originally from Brazil. Earlier research identified a particularly high use of diet pills among Brazilian-born women.
Twenty-three percent reported using Pai You Guo, in either capsule or pill form. Eighty-five percent reported at least one side effect, including dry mouth, anxiety and insomnia. About 66 percent of users purchased Pai You Guo from acquaintances, 30 percent from stores and 9 percent over the Internet. Most participants bought the supplement after the recall.
People take Pai You Guo thinking they're using a natural weight loss remedy. "Instead, they're consuming banned pharmaceutical product," Cohen said.
Pai You Guo has been found to contain sibutramine, a weight loss drug that was recalled in 2010 due concerns it might increase the risk of heart attack and stroke. Some capsules and teas containing Pai You Guo have been found to contain doses of sibutramine six times higher than the doses found in prescription pills, Cohen said, and the substance is not even listed on the product label.
This problem likely extends beyond the Boston area, Cohen said. Pai You Guo has been reportedly sold in California, Florida and Washington state. "We're talking about a national problem," Cohen said.
Others say the problem is likely not common in the general population. It is likely confined to certain pockets of the population, said Dr. Bimal Ashar, an associate professor of medicine at John Hopkins School of Medicine in Baltimore.
Rotten tomatoes?Congress should re-write the rules that govern dietary supplements, Cohen said. The FDA should be given the authority to approve dietary supplements before they go to market, Cohen said, and should ensure the drugs are safe and really contain what's on the label, he said.
Right now, "supplements are regulated by the FDA as if they are food," Cohen said. But unlike, say, a spoiled tomato, "the consumers has absolutely no ability when they're popping a pill to know if it's spoiled or rotten or not," Cohen said.
Ashar agreed regulation of weight loss supplements should be enhanced. And the FDA, he said, should increase awareness about the way these products are regulated.
"I don't think the public really knows that this stuff is not regulated," before it comes to market, Ashar said. "They think ok, this is in a bottle…it must be pure, it must be fine to take."
"There needs to be a very, very highly publicized campaign about dietary supplement," in which the FDA says what it can do and will do to regulate these substances, Ashar said. However, even if better regulations are put in place, it may be tough to prevent the sale of these weight loss supplements if most are acquired through acquaintances, Ashar said. The FDA would not have the resources to track down such sales the way law enforcement tracks down the sales of narcotics, making it practically impossible, Ashar said.

Fecal Occult Blood Tests Can Do More Harm Than Good in the Elderly

Among elderly men, 87% with the worst life expectancy and 65% with the best life expectancy experienced more burden than benefit from fecal occult blood testing. The U.S. Preventive Services Task Force recommends that colorectal cancer (CRC) screening be discontinued after age 85  (Incorrect! Don't think one can't get colon cancer after the age of 85!) and that decisions to perform screening be individualized for patients aged 75 to 85. Several other guidelines recommend against CRC screening if individual life expectancy is <10 years="" but="" supporting="" evidence="" is="" mostly="" from="" short-term="" follow-up="" studies="" to="" assess="" the="" benefit="" versus="" burden="" of="" fecal="" occult="" blood="" test="" fobt="" screening="" in="" elderly="" researchers="" conducted="" a="" 7-year="" longitudinal="" study="" involving="" 212="" patients="" age="" range="" 70="" 84="" 99="" 5="" men="" who="" received="" positive="" results="" at="" four="" veterans="" affairs="" medical="" centers="" net="" was="" defined="" as="" detection="" crc="" or="" significant="" adenoma="" 1="" cm="" 3="" more="" adenomas="" an="" with="" villous="" features="" followed="" by="" treatment="" and="" survival="" for="" encompassed="" false-positive="" tests="" deaths="" within="" undetected="" cancers="" when="" declined="" analyzed="" status="" life="" expectancy="" which="" they="" classified="" follows="" using="" charlson-deyo="" comorbidity="" index="" cci="" measure="" comorbidity:="" br="">

•              Best: age range, 70–79 and CCI, 0 (life expectancy, >10 years)

•              Average: age range, 70–84 and CCI, 1–3; or age ≥80 and CCI, 0 (life expectancy, 5–10 years)

•              Worst: age range, 70–84 and CCI ≥4; or age ≥85 and CCI ≥1 (life expectancy, <5 years="" span="">

Of all participants, 16% experienced net benefit from FOBT screening, and 70% experienced net burden; benefit status was indeterminate in 14%. The percentage of patients who experienced a net burden varied by life expectancy: best, 65%; average, 70%; and worst, 87%. Net benefit also varied by life expectancy: best, 20%; average, 15%; and worst, 10%.??Comment: This study is somewhat hampered by ambiguous definitions of screening burdens. For example, a net burden from FOBT screening in patients who had cancer but did not undergo colonoscopy would seem to result from failure to follow up the positive screening test, not the screening test itself. Appropriate follow-up would have shifted a net burden to a net benefit. Also, although a positive FOBT without a follow-up colonoscopy in a person without cancer might constitute a net burden, the burden seems minimal. Nevertheless, the study provides some quantitative data on CRC screening benefit by patient life expectancy. These issues might be even more relevant to colonoscopy screening, since the costs and risks of colonoscopy and polypectomy are incurred at the initial decision to screen. However, with FOBT screening, a positive test can still be followed by a decision to forgo colonoscopy if substantial morbidities or advanced age are factors. In that case, of course, the initial decision to perform the FOBT was probably inappropriate.??— Douglas K. Rex, MD Published in Journal Watch Gastroenterology June 3, 2011

Medicine continues to be an art more than a science. For years, we have been telling patients to check for occult (hidden) blood in their stools as a screening for colon cancer. The truth? MOST cancers and precancerous polyps don’t ever bleed! The only and best way to screen and prevent colon cancer is to get colonoscopies on a regular basis depending on risks factors. That would be family history and previously history of polyps. I have stopped doing the hidden blood testing 11 years ago when I realized too many lesions were being missed on this basis. Sure, the prep is awful, but the anesthetic makes you feel wonderful and it is painless.                                                                    - Harvey S. Mishner, M.D.

Fish Farming Doesn't Help Endangered Resources

Kaizen Dietitian Barbara Bingham discusses the question of wild versus farmed fish.

I choose 'wild-caught' fish and shrimp because of research I've seen which found toxins, antibiotic residues and other disease-controlling drugs in fish and shrimp raised in extremely crowded farm conditions. If large quantities of chemicals were not used the farm fish would die.  (Thailand and other Asian farms are more dangerous than domestic conditions.)  As well as these substances ending up in the fish or shrimp you eat, the farm contaminates surrounding waters, fresh or ocean.  Because of concern about overfishing (which has threatened many species) some people assume eating farm-raised salmon, for example, is a way of protecting the wild population.  However, it takes several pounds of feed fish to produce one pound of salmon. The net result of salmon farming is greatly accelerated depletion of the numbers of fish in the oceans. Besides, farmed salmon has less flavor, less protein, and more fat than wild salmon, and its content of omega-3-fatty acids is not quite as high.  Look at your seafood counter or on the label of frozen products and it will clearly say "wild" or "wild-caught'.

Colored Contact Lenses Give 14 Year Old Corneal Transplant

A 14 year old girl may need a corneal transplant after buying and wearing $20 colored contact lens.

Federal law prohibits the sale of contacts without a prescription. However beauty shops and street vendors sell them to customers very inexpensively for cosmetic purposes.

So, what does this have to do with Kaizen Total Wellness and our adult patients?

Beware what you buy without FDA approval-from supplements to vitamins, even internet prescriptions.
