Dr. Jill Rose, Physical Therapist at Kaizen Total Wellness Center comments on article in Fox News concerning knee replacement surgery and patient disappointment with results:
Nearly 80% of people over the age of 75 have some form of osteoarthritic (OA) changes seen on x-ray. Symptoms include pain, stiffness, inflammation, instability, deformity, and loss of function. Fox news recently released a study from West Virginia Unviersity discussing the disappointments that people have following knee replacement surgery. The study found that people had hoped to be able to return to doing an average of 23 hours a week of moderate activity but were only able to return to 11 hours following surgery. Although this was greater than the average of 2 hours a week they were able to do prior to surgery it was significantly less than they expected. Although knee replacement surgery may be the only option for some who have severe arthritis in their knees there are other conservative options that may be tried first.

Your muscles will feel more energized after 20 minutes on this vibrating surface.

It’s 1:12 PM, and I’m still on the road. My appointment at Bethesda’s Equinox Gym was supposed to begin at 1, but I’m still inching up Wisconsin Avenue (It’s a Wednesday, people—aren’t you supposed to be at work?!) and in a pretty awful mood. My muscles ache with dull pain from sitting in an office chair for nine hours a day (and in traffic for the past hour and a half), and now my only workout for the week has been cut down by almost a full 15 minutes.??After screeching into a nearby parking garage, I frantically change in the front seat (please tell me after-the-fact confessions can’t get you arrested for public nudity) and sprint over to the facility. Equinox’s smiling Joe Sigismondo whisks me upstairs. At 1:21, I finally come face to face with the reason I volunteered for this trek in the first place: the Power Plate.
The Power Plate, originally developed to help astronauts rapidly gain muscle, is a vibrating platform large enough to accommodate the average person performing a deep squat. Since it was developed for commercial use in 1999, celebs such as Madonna, Kelly Osbourne, and Courtney Cox have gushed about it being a quick way to get in an intense workout—and I’ve been dying to try it.??As you perform basic exercises such as push ups, stretches, and squats on the plate, the targeted muscles involuntary contract at a rate of 25 to 50 times per second. This helps maintain balance in reaction to the vibration, thereby causing increased blood flow and deeper stimulation.??If you’ve ever been victim to electroshock therapy while recovering from a knee injury, the sensation of the Power Plate is similar, but much less intense. Plus it spreads past the area being targeted into muscles you wouldn’t normally work when performing standing calisthenics.??There are three different settings on the machine: hertz (vibrations per second), amplitude (height of vibration), and time. As a first-timer, Joe recommends I start off on 30-30-low—that’s 30 vibrations per second, with the platform moving mere millimeters up and down. We begin with basic stretches like lunges and a position similar to child’s pose to stretch my shoulders, holding for 30 seconds at a time.
After these short bursts, my hamstrings are already tingling the way they would after 15 minutes on a treadmill. Joe mentions the Power Plate is a great way to warm up before a cardio session, since it loosens muscles and increases blood flow far more effectively than traditional standing stretches.??Next, we work through two sets of step-ups, squats, planks, and modified V-sits. Once my body realizes that the key to feeling the full effects of the vibrations is to perform slow, controlled movements, and to hold my position—no matter how hard my muscles are moving—I begin to understand why one Marie Claire staffer wrote that she felt “completely drained” after just 20 minutes on the thing. I’m not quite there yet, though, so Joe turns my timer up to 45 seconds, and we start on push ups and cable rows for my arms. After the second set of push ups, I’m almost ready to collapse; it’s only been a minute and a half, and my arms feel more engaged than after a half-hour session on the weight machines.??Lastly, Joe walks me through two “massages.” My favorite is for the trapezius muscle. For this massage, Joe tells me to sit on the floor with my back against the base of the plate and hold the cable rows attached to the machine tightly across my body. I feel the vibration deep in my shoulders, and the ache of hunching over a keyboard all day seems to melt away.

From skipping rope to sprinting - at some point in our lives we have performed some type of plyometric activity. Plyometric training is a fancy way of saying you did something really quick. Athletes capitalize on this mode of exercise all the time - inclusive in Power training. Unfortunately, most athletes and the general populace have no knowledge of "true" plyometric training. Some have gotten wind of this exercise phenomenon through so-called "jump" training. While true that plyometrics include jumping, not all jumps are plyometric! The Russians implemented "shock training" to their athletes and military personnel decades ago. In the 50's some forward thinking soviets introduced the West to this specialized training and from there the misconceptions grew. Plyometrics are meant to be powerful and explosive with less than 2/10s of a second of contact (ground reaction time) - literally in the blink of an eye. Since the easiest employment came through jumping, most saw jumping as plyometric and incorrectly began jumping through hoops and such to showcase this new-fangled exercise. The fact is, a plyometric exercise is a muscle action capitalizing on the stretch-shortening cycle or SSC. This can come in the form of throwing, running, jumping, clapping ... or PowerPlate training at Kaizen Total Wellness.
Here is the beauty of the PowerPlate machine at Kaizen - to create a plyometric movement there must be a counter-movement to initiate and employ the elasticity of the muscle and thereby utilize the SSC. For instance, to jump we inherently squat (even a little) and then triple extend about the hip, knee and ankle joints. Ask anyone to jump and you will see this occur everytime, if not the jump is less than impressive. So, what if you should not jump or just do not want to jump? What if you wish to train the upper body instead of the legs? Your answer is the PowerPlate! Why? The Vibration technology creates a reflexive neuromuscular response in the body. Meaning, you do not need to consciously contract your muscles, the machine transmits energy into the body and the body responds instinctively. Science has shown that a reflexive action is quicker and more powerful than a conscious effort (think Fight-Flight-Fright reflex intrinsic in all mammals). On a subtle, continuous and SAFE level, the platform drops from under foot and causes a reflex response. My name is Jim Pfrogner [age 69] and I have been training with the Power Plate for about ten weeks. I have an arthritic problem in my lower legs and it has affected my quality of life. We have been using this technology at Kaizen Total Wellness® for the past 1 1/2 years with a great deal of success! We are currently the only facility in Florida offering this combination of technologies. BStrong4Life™, a Chicago-based wellness company has launched a 6-month pilot study to determine the effectiveness of their system on improving bone mineral density without medication. The system is a combination of two technologies: Power Plate, and bioDensity, as well as a proprietary training system that integrates the two.
Power Plate equipment works on the principles of whole-body vibration. This proprietary system uses warm-up procedures to prepare the user for each bioDensity session and to optimize performance. The Power Plate equipment vibrates almost imperceptibly, causing whole body vibrations that create instability and trigger reflexive contractions in the muscles, as frequently as 25 to 50 times per second.
The equipment also oscillates, so the muscles must contract in multiple dimensions. This technique stimulates the fast-twitch muscle fibers, which can lead to increased muscle strength, endurance and overall agility. At both Kaizen Total Wellness and in my medical practice we try to get all of our patients to lead a more active lifestyle. Whereas 15 minutes of moderate exercise increasing life span by 3 years doesn’t seem that impressive, we find that the quality of life is dramatically improved both physically and mentally. We like our patients to try to do aerobic activities (walking, biking, swimming, etc.) for at least 30 minutes everyday and weight resistance 2-3 times a week. Our low impact weight resistance programs are safe for seniors and 15 minutes twice a week with our fitness trainers is equivalent to spending 1 hour three times a week in a conventional gym.

Fifteen Minutes of Moderate Daily Exercise Lengthens Life, Taiwanese Study Finds
ScienceDaily (Aug. 17, 2011) — Taiwanese who exercise for 15 minutes a day, or 92 minutes per week, extended their expected lifespan by three years compared to people who are inactive, according to a study published in The Lancet"Exercising at very light levels reduced deaths from any cause by 14 percent," said study senior author Xifeng Wu, M.D., Ph.D., professor and chair of The University of Texas MD Anderson Cancer Center Department of Epidemiology. "The benefits of exercise appear to be significant even without reaching the recommended 150 minutes per week based on results of previous research."
Lead author Chi-Pang Wen, M.D., of the National Health Research Institutes of Taiwan, and colleagues also found that a person's risk of death from any cause decreased by 4 percent for every additional 15 minutes of exercise up to 100 minutes a day over the course of the study. Those exercising for 30 minutes daily added about four years to life expectancy.
"These benefits were applicable to all age groups, both sexes and those with cardiovascular disease risk," the authors note.
If inactive people in Taiwan were to do low-volume daily exercise, one in six deaths could be postponed by their reduced risk of dying, the authors report. It would be an estimated reduction in mortality similar to that from a successful tobacco control program.
The prospective observational study involved 416,175 Taiwanese who participated in a standard medical screening program run by MJ Health Management Institution between 1996 and 2008. Participants were followed for an average of eight years.
For the exercise study, participants completed a questionnaire covering their medical history and lifestyle information. They characterized their weekly physical activity for the previous month by intensity -- light (walking), moderate (brisk walking), vigorous (jogging) or high vigorous (running) -- and time.
To account for occupational effects, participants also characterized their physical activity at work, ranging from sedentary to hard physical labor.
Those who reported less than one hour a week of leisure time physical activity were classified as inactive -- 54 percent of all participants. Others were classified as low, medium, high or very high based on the duration and intensity of their exercise. Researchers calculated mortality risk and life expectancy for each group.
Thirteen other variables were analyzed: age, sex, education level, physical labor at work, smoking, alcohol use, fasting blood sugar, systolic blood pressure, total cholesterol, body mass index, diabetes, hypertension and history of cancer.
Those who engaged in low-volume exercise had lower death rates than inactive people regardless of age, gender, health status, tobacco use, alcohol consumption or cardiovascular disease risk.
The researchers note that the World Health Organization and the U.S. Centers for Disease Control and Prevention recommend at least 150 minutes of moderate-intensity exercise per week. A third of U.S. adults meet that guideline; about 20 percent of adults in China, Japan or Taiwan meet it.
"A recommendation of 15 minutes of daily exercise should be promoted to East Asian populations," the authors note.
The study's findings of reduced mortality through even moderately intense exercise are likely to hold true for other populations, Wu said, even though the total amount of time spent or workout intensity required for a health benefit might differ. "These findings can stimulate people to exercise as much as they can and to not be frustrated that they can't reach the 30 minute per day guideline."
This is the first collaboration between Wu, Wen and the MJ Health Group, a major health screening company with more than 600,000 participants in its health database. They have formed the Asian Health Screening Cohort to conduct major research projects. Wu provides scientific expertise with Wen, who also is based at China Medical University Hospital, while MJ Health Group contributes patient epidemiological and clinical data as well as a biobank of tissue samples.
Two other ongoing collaborative projects include development of a liver cancer risk prediction model and a study of telomere length, genetic variation and cancer risk. The second project is funded by an MD Anderson Sister Institute Network Fund Grant. MD Anderson and China Medical University Hospital have a sister institution agreement.
The exercise project was funded by the Taiwan Department of Health Clinical Trial and Research Center of Excellence and the Taiwan National Health Research Institutes.

A recent study in JAMA suggests the long term use of Bisphosphonate drugs may actually increase the risk of thigh fractures, especially use for greater than five- years.
In our practice we see many patients fail to achieve any meaningful bone strengthening even after 10 years of medications! Active weight resistance exercising along with adequate Calcium and Vitamin D seems to work as well and is much healthier.
